Participation Options
1.) By internet - To access the Bath Township Community Survey online, visit www.bathtownship.org and click on the "Community Survey" link located on the homepage OR scan the QR code at the bottom of this postcard.
*The unique identifier code listed on this postcard near your address will be
required. Please limit responses to two per household.
* Please note the acronyms UIC and UIN are interchangeable.
2.) By in-person drop off - If you do not have access to the internet, a hard copy of the community survey may be obtained in the Township Administration Office, Monday-Friday, 8:00 am to 4:00 pm.
3.) By phone - A representative is available over the phone for residents unable to complete the survey through an additional method. Please call 330-666-4007 Monday-Friday, 8:00 am to 4:00 pm.
